MHBP Standard Option and Value Plan members can earn Wellness Fund account rewards for completing a health risk assessment (HRA) and a biometric screening. The rewards can be earned once per calendar year for each covered family member ages 18 and older. You can use them to help pay for qualified medical expenses, such as your cost-sharing amounts (copayments, coinsurance and deductibles) for future services, for any covered family members.

Earn up to a $100 credit for completing an HRA

An HRA can help individuals identify potential risks to their physical and mental health. The HRA starts with a questionnaire that asks about your nutrition, weight, physical activity, stress, safety and mental health, kind of like an interview. Your responses can lead to suggestions and programs that can help you improve your health by reducing risks. After you complete the questionnaire, you’ll get a personalized summary that helps you identify and understand potential risks.

MHBP offers a free and confidential HRA online. To take the HRA, log in to your Aetna Member Website, select Discover A Healthier You and proceed. If you haven’t logged in before, you’ll need to register for a member account.

If you prefer to complete the HRA by phone, call us at 1-866-533-1410 (TTY: 711). We’ll help you schedule an appointment so a health coach can assist you with completing the HRA.

After you complete the HRA, you’re eligible to receive a $100 (Standard Option) or a $75 (Value Plan) credit to your Wellness Fund account. You can use this credit for qualified medical expenses, such as your cost-sharing amounts for future services. The reward is available once per calendar year to all members ages 18 and older and can be used by any covered family member.

After you have completed your HRA, we will credit your Wellness Fund account with your reward.

Earn Up to a $100 Credit for Completing a Biometric Screening

A biometric screening measures certain physical characteristics: height, weight, body mass index, blood pressure, blood cholesterol, blood glucose, and aerobic fitness. It creates a benchmark for you to evaluate changes in your health status over time. Your doctor can do one as part of your annual physical exam, or you can have a biometric screening done – at no cost to you – at a Quest Diagnostics Patient Service Center. When your results are returned by November 30, you’ll earn a Wellness Fund account reward of $100 (Standard Option) or $75 (Value Plan).

How to earn your biometric screening reward:

  • Schedule your screening at a Quest Diagnostics location or request a home kit. Call 1-855-623-9355 or visit My.QuestforHealth.com and enter the registration key: MHBP to register for your screening or to request a home kit.
  • Have your doctor perform the screening as part of your annual check-up and record the results on the Biometric Screening Physician Results form. Then fax the form to Quest Diagnostics no later than November 30. The Biometric Screening Physician Results form and instructions are available at My.QuestforHealth website.

Once your biometric screening is complete, your results will be available online at My.QuestforHealth website.

After you have completed the biometric screening, we will credit your Wellness Fund account with your reward.

Healthy Actions

By completing healthy actions, you’re eligible to earn $50 that’s deposited into a Wellness Incentive Fund account. You can use this credit for qualified medical expenses, such as your cost-sharing amounts for future services. Healthy actions include:

Controlling blood pressure for members with high blood pressure

The plan will reach out to you if you’re identified through claims data as having high blood pressure. We’ll provide you a form for your provider to complete. On the form, your provider must document two (2) controlled blood pressure readings below 140/90 on separate visits during the current calendar year. Then you’ll earn the incentive.

If you’re unable to meet this goal, you’ll earn the incentive if one of the following is completed by December 1 of the calendar year:

  • Telephonic Health Coaching Program (Tobacco/e-cigarette cessation, Weight Management, Exercise, Nutrition, Stress Management)
  • Dietary and nutritional counseling (obtain three counseling visits, which includes individual and group behavioral counseling) (See Section 5(a) Preventive Care, adult in your official Plan Brochure)

Controlling A1c Hemoglobin levels for members with diabetes

The plan will reach out to you if you’re identified through claims data as having diabetes and ask you to have your provider submit your A1c laboratory results. Your A1c laboratory results must be less than 8% during the calendar year for you to earn the incentive.

If you’re unable to meet this goal, you’ll earn the incentive if one of the following is completed by December 1 of the calendar year:

  • Dietary and nutritional counseling (obtain three counseling visits, which includes individual and group behavioral counseling)
  • Diabetic Education or Training

Maternal Wellness

Complete any of the following steps to earn a reward:
•Enroll in our Maternity Program or complete a pregnancy survey by the 16th week of your pregnancy. – $25
•Send provider documentation of prenatal visit in 1st trimester – $25
•Complete pre-delivery call between 27-32 weeks with a Care Manager – $25
•Complete a 4-week postpartum call with a Care Manager – $25

Members ages 18 or older can earn financial incentives through participation in the Health Risk Assessment, Biometric Screening and Wellness Incentive’s Programs. These incentives are deposited into a Wellness Fund Account and can be used by any covered family member. They’re available once per calendar year.

Note: Standard Option members are limited to $350 per person per calendar year. Value Plan members are limited to $300 per person per calendar year.

Wellness incentive rewards are available for reimbursement for Qualified Medical Expenses as defined by IRS Code 213(d). These expenses include, but are not limited to, services you or your covered dependents incur, medical plan deductibles, diagnostic services covered by your plan, long-term care premiums, health insurance premiums if you’re receiving Federal unemployment compensation, physician prescribed over-the-counter drugs, LASIK laser eye surgery, and some nursing services. If you enroll in Medicare, you can use the account to pay Medicare premiums. For a detailed list of IRS-allowable expenses, request a copy of IRS Publication 502. To do so just call 1-800-829-3676, or  visit the IRS website and click on “Forms and Publications.” Note: Although physician prescribed over-the-counter drugs are not listed in the publication, they are reimbursable from your Wellness Fund Account.

Wellness incentives are automatically issued when a medical claim is processed that results in member liability (e.g. copay or deductible). Members can also submit a claim to request reimbursement from their Wellness Fund Account by completing the Wellness Fund Reimbursement Request Claim Form.

Note: If you are enrolled in a Flexible Spending Account (FSA) and wellness incentives have been deposited into your Wellness Fund Account, you may not receive reimbursement for the same medical expense from both your Wellness Fund Account and your FSA. If a medical expense is covered under both your Wellness Fund Account and your FSA, your Wellness Fund Account will be used first. You may receive reimbursements from your FSA for medical expenses that are covered by both your Wellness Fund Account and your FSA only after the funds in your Wellness Fund Account have been used up.
