Healthy actions
Standard Option and Value Plan members can earn a $50 credit for the following healthy actions**
- Controlling blood pressure for members with high blood pressure
- Controlling A1C hemoglobin levels for members with diabetes
Earn your wellness rewards
You can earn up to $200 (Standard Option) and $150 (Value Plan) for qualified medical expenses by completing your health risk assessment (HRA) and biometric screeningy
Health Risk Assessment (HRA) credit — up to $100
Finding potential health risks can help you manage your health. It’s as easy as answering a few questions about your nutrition, weight, physical activity, stress, safety and mental health.
You can complete your HRA on your Aetna member website:
- Select “Discover a Healthier You” under “Health & Wellness”
- Hover over “My Health” on the top navigation bar
- Choose “Health Assessment”
- Complete questions
- Select “Submit Now” once all answers are entered
Once complete, you’ll get a custom summary that can help you identify and understand potential health risks and will suggest programs that can help you improve your health. Best of all, you’ll earn a $100 credit (Standard Option) or a $75 credit (Value Plan) to your wellness fund account.
Consumer Option members are not eligible to earn credits for completing their HRA or Biometric Screening.
You can also complete your HRA over the phone.
Just call 1-866-533-1410 (TTY:711) to set up an appointment with a health coach.
Complete a biometric screening through Quest Diagnostics or Labcorp and receive a Wellness Fund Account incentive reward
Checking your biometric numbers is a great way to monitor your health.
To register for a screening through Quest Diagnostics, call 1-855-6BE-WELL (1-855-623-9355) or visit My.QuestForHealth.com and enter registration key: MHBP.
Or, you can have your physician perform the biometric screening as part of your annual checkup. Then record the results on the Biometric Screening Physician Results form and fax it to Quest Diagnostics.
Go to My.QuestForHealth.com to access the form.
With either option, you’ll earn a $100 credit (Standard Option) or a $75 credit (Value Plan) to your wellness fund account.
To register for your screening at a Labcorp PSC, or have your physician record results on the PCP form, or to order your OnDemand home test kit, call 844-251-6524 for assistance or visit WellConnect Plus.
Your results may be viewed on the WellConnect Plus website once your screening is complete.
Consumer Option members are not eligible to earn credits for completing their HRA or Biometric Screening.
Maternal Wellness
Complete any of the following steps to earn a Maternal wellness reward:
- Enroll in our Maternity Program or complete a pregnancy survey by the 16th week of your pregnancy – $25
- Send provider documentation of prenatal visit in 1st trimester – $25
- Complete pre-delivery call between 27-32 weeks with a Care Manager – $25
- Complete a 4-week postpartum call with a Care Manager – $25
Consumer Option members are not eligible to earn Maternal Wellness rewards.

How to use Wellness rewards
You can use your wellness funds to pay for medical expenses incurred by you or your covered dependents. Expenses include, but are not limited to:
- Coinsurance, copayments and deductibles
- Dental services such as crowns, bridges, extractions and orthodontia
- Vision services such as eyeglasses and contact lenses
*To monitor the availability of funds in your Wellness Incentive Fund Account, visit the Plan’s website (MHBP.com), and then select “Your Aetna Member Website.” Once you log in, look for the tab, “Discover a Healthier You” under “Health and Wellness” and proceed. Please allow up to eight weeks after completion for the incentive to show in your account.
**Standard and Value Plan members can submit a claim for reimbursement of a qualified medical expense. The wellness incentive account reimbursement claim form can be found at MHBP.com in “Official Plan Documents” under the Member Resources section.
To receive your incentive for any of the above noted healthy actions, you must submit the required documentation by December 31 of the calendar year to the following address:

Members ages 18 or older can earn financial incentives through participation in the Health Risk Assessment, Biometric Screening and Wellness Incentive’s Programs. These incentives are deposited into a Wellness Fund Account and can be used by any covered family member. They’re available once per calendar year.
Note: Standard Option members are limited to $350 per person per calendar year. Value Plan members are limited to $300 per person per calendar year.

Wellness incentives are automatically issued when a medical claim is processed that results in member liability (e.g. copay or deductible). Members can also submit a claim to request reimbursement from their Wellness Fund Account by completing the Wellness Fund Reimbursement Request Claim Form (PDF).
Note: If you are enrolled in a Flexible Spending Account (FSA) and wellness incentives have been deposited into your Wellness Fund Account, you may not receive reimbursement for the same medical expense from both your Wellness Fund Account and your FSA. If a medical expense is covered under both your Wellness Fund Account and your FSA, your Wellness Fund Account will be used first. You may receive reimbursements from your FSA for medical expenses that are covered by both your Wellness Fund Account and your FSA only after the funds in your Wellness Fund Account have been used up.
