If you are managing a special medical condition such as asthma, diabetes, chronic renal failure, congestive heart failure or others our Care Support Program can help. The goal is to help you better understand and take charge of your medical condition by:

• Providing you with condition-specific health care information
• Contacting you periodically to see how you're doing
• Discussing your treatment with a doctor or caregiver
• Suggesting coping strategies
• Explaining techniques to help you and your doctor effectively monitor your medical condition

This program is part of the MHBP benefit package, and your participation is optional. If you register for the program, you may discontinue your participation at any time. URAC-Accredited Disease Management Programs

MHBP offers URAC-Accredited Disease Management Program for six conditions: Congestive Heart Failure, Coronary Artery Disease, Chronic Obstructive Pulmonary Disease, Asthma, Diabetes and Chronic Kidney Disease.
