Care Support Program Rights & Responsibilities


As a participant in the Care Support Program, you have the right to:

 

  • Receive accurate, easily understood information about our Care Support Program, including our philosophy and features of our services.

 

  • Receive considerate, respectful assistance from all our representatives at all times and under all circumstances, know the identity of our representatives by first name, and speak to a supervisor if requested. An environment of mutual respect is essential to maintain a quality health benefits system.

 

  • Communicate with us in confidence and have the confidentiality of their identifying health care information protected.

 

  • Fully participate in all decisions related to their health care. Members who are unable to fully participate have the right to be represented by parents, guardians, family members or others as designated.

 

  • Receive information regarding changes in or termination of our Care Support Program.

 

  • Decline or terminate participation at any time in our Care Support Program.

 

As a participant in our Care Support Program, you have the responsibility to:

 

  • Accurately and completely disclose relevant information and notify us of any changes.

 

  • Become involved in individually specific health care decisions.

 

  • Notify treating health care providers, as applicable, about participation in the disease management program and share program materials.

 

  • Work collaboratively with health care providers in developing and carrying out agreed-upon treatment plans.

 

  • Make a good-faith effort to maximize healthy habits, such as exercising, not smoking, and eating a healthy diet.

 

  • Abide by the administrative and operational procedures of our Care Support Program.
